The enquiry form on this website opens your own email client with a pre-filled message. That means the details you enter are transmitted to us as an ordinary email from your own email account — we do not store them on this website.
Please do not submit patient data, protected health information, financial account details, government identification numbers or any other sensitive information through this website or by unencrypted email. If sensitive information needs to be exchanged, we will arrange a secure channel first.
